Registration With The Practice
When registering with the practice, we ask that you bring your medical card or documents showing your NHS number and place of birth along with details of your previous general practice (GP). You will also be asked to provide proof of address. (Please see the next tab, 'Your NHS Number' for more details on this).
We request new patients complete the GMS1W and Additional Information forms (see below links) and if appropriate, the Carers Identification form and /or the Dispensary Request form.
On receipt of the completed forms we will register you with our practice. We recommend you make an appointment to see one of our Health Care Assistants for a New Patient Health Check. This health check will provide us with important details about you pending receipt of full records from your previous GP. We do this as it can take a considerable time for us to receive your medical records. If you are currently on repeat medication we will also need to see your last prescription slip.
To save time, you can print the Registration Forms below, complete, sign and return it to the surgery in person when you register, along with your proof of identify and proof of address. Alternatively, you may ask for a Registration Pack at Reception and we will be happy to provide one for you to complete.
**IMPORTANT- PLEASE COMPLETE THE NEW PATIENT ADDITIONAL INFORMATION FORM FOR EACH APPLICANT AS IT INCLUDES VITAL INFORMATION. FAILURE TO SUBMIT THIS MAY RESULT IN A DELAY IN REGISTRATION AT THE PRACTICE**
